How to Register with SSS - For Employed Individuals

FOR NEW EMPLOYEES

It is compulsory for all prospective and/or new employees to register under SSS. This will also ensure that you get the benefits as a member of the Philippine society, in cases such as maternity, sickness, calamity and others.

When I had to go thru D&C (raspa due to blighted ovum), I got a whopping P30,000 for my maternity benefit! It was such a huge help since hospitalization is not that cheap. The benefits, though are based on how much your contributions are and how long you have been a contributing SSS member.

Who are covered?

  • A private employee, whether permanent, temporary or provisional, who is not over 60 years old.
  • A household-helper earning at least P1,000 a month is compulsory covered starting Sept. 1, 1993.
    • A household-helper is any person who renders domestic services exclusively to a household such as a driver, gardener, cook, governess, and other similar occupations.
  • A Filipino seafarer upon the signing of the standard contract of employment between the seafarer and the manning agency which, together with the foreign ship owner, act as employers.
  • An employee of the foreign government, international organization or their wholly-owned instrumentality based in the Philippines, which entered into an administrative agreement with the SSS for the coverage of its Filipino workers.
How to Register with SSS - Steps and Requirements

Registering as a member under SSS is very easy, but you have to be patient as there are a lot of people lining up for registration. I, personally, registered at the Makati - Gil Puyat Branch and it wasn't that crowded, same goes with the Manila branch near SM Manila. I think the main branch in Quezon City is too crowded so I won't suggest that you go there. For a list of SSS offices or branches, just go to www.sss.gov.ph and click on the drop-down button under "Branch Directory", as shown in the picture below:
  • Accomplish SSS Form E-1 (Personal Record) in two (2) copies and submit at any SSS office or branch.
    • You can get this form at any SSS office or branch.
    • You can also download this online and print it (make sure to cut along the dotted line. To download the SSS Form E-1, just click the link below, right click on the document and click "Print": https://www.sss.gov.ph/sss/uploaded_images/forms/normal/e1.pdf (The SSS E-1 Form is in page 1, while page 2 lists a summary of the requirements and some reminders.)
    • Here is a sample of the SSS E-1 Form:
  • PRIMARY DOCUMENTS NEEDED
Submit original/certified true copy AND photocopy of ANY of the following:
  • baptismal certificate
  • birth certificate
  • driver's license
  • passport
  • Professional Regulation Commission (PRC) Card or Seaman's Book
If you don't have the above-mentioned documents, ANY TWO of the following, one of which with recent photo and date of birth, are acceptable:
  • Alien Certificate of Registration
  • ATM card with the cardholder’s name
  • ATM card and certification from the issuing bank that the account number belongs to the cardholder, if the card does not bear the cardholder’s name.
  • Bank Account Passbook
  • Birth/Baptismal Certificate of children
  • Certificate form
    • Office of the Southern/Northern Cultural Communities
    • Office of Muslim Affairs
  • Certificate of Naturalization issued by the Bureau of Immigration
  • Company ID
  • Company Representative Authorization Card issued by SSS
  • Credit Card
  • Fishermen’s Card issued by Bureau of Fisheries & Aquatic Resources (BFAR)
  • GSIS Card
  • GSIS Certificate of Membership
  • Health or Medical Card
  • ID Card issued by Local Government Units (e.g. Barangay/     Municipality/City)
  • ID Card issued by professional associations recognized by PRC
  • Life Insurance Policy
  • Marriage Contract
  • NBI Clearance
  • Overseas Workers Welfare Administration (OWWA) Card
  • Pag-Ibig Member’s Data Form
  • Permit to Carry Firearms issued by the Firearms & Explosives Unit of the Philippine National Police (PNP)
  • PHIC Member’s Data Record
  • Police Clearance
  • Postal ID Card
  • School ID
  • Senior Citizen Card
  • Tax Identification Card/Affidavit
  • Transcript of School Records
  • Voter’s Identification Card/Affidavit
  • ADDITIONAL DOCUMENTS:
    • For Married individuals - marriage contract
    • If with reported children:
      • if with legitimate child/ren - birth or baptismal certificate/s of children
      • if with legitimated child/ren (e.g. legitimited due to subsequent marriage of parents) - birth certificate with annotation "legitimated"
      • if with legally-adopted children - decree of adoption
      • if with illegitimate children - birth or baptismal certificate
    • ***If you don't have the above-mentioned documents, ANY of the following stating the relationship to the reported dependent/beneficiary should do:
      • School record
      • Insurance Policy
      • Statement before a court
***IMPORTANT REMINDERS: 

  • Be early at the SSS branch so you can be ahead of the line.
  • Make sure that you bring with you ORIGINAL or CERTIFIED TRUE COPIES of the documents when you register at SSS, for authentication purposes, AND PHOTOCOPIES of those documents.
  • After registration, keep your SSS Form E-1 and take note of your SSS number, so you don't have to go thru SSS number verification in case you lose your E-1.
  • Issuance of SS number does not automatically qualify you for coverage. You must first be engaged in an occupation subject to SSS coverage.
  • An SS number is a lifetime number. No one should have more than one SS number.
  • You need to have at least one (1) month of SSS contribution before you can apply for an SSS ID or UMID.
Once you have submitted all the requirements, the SSS personnel will give you your SSS number and you are officially a member of the SSS! Yey!
