How to Register with SSS - For Employers

FOR EMPLOYERS


Who Are Covered?



  • An employer, or any person who uses the services of another person in business, trade, industry or any undertaking.
    • A social, civic, professional, charitable and other non-profit organization which hire the services of employees are considered "employers".
  • A foreign government, international organization or its wholly-owned instrumentality such as embassy in the Philippines, may enter into an administrative agreement with the SSS for the coverage of its Filipino employees.

Steps and Requirements:

Single Proprietorships   
An owner of a single proprietorship business should accomplish and submit SSS Form R-1 (Employer Registration) and R-1A (Employment Report).

Partnerships   

Any of the partners of a partnership firm should accomplish SSS Form R-1 (Employer Registration) and SSS Form R-1A (Employment Report) and submit these forms together with a photocopy of the Articles of Partnership. The original copy of the Articles of partnership MUST be presented to the SSS for Authentication purposes.

Corporations   

A corporation must accomplish SSS Form R-1(Employer Registration) and R-1A (Employment Report) signed by its President or any of the corporate officers and submit these forms together with a photocopy of the Articles of Incorporation. The original copy of the Articles of Incorporation MUST be presented to the SSS for authentication purposes.

Household Employers   

A household employer should accomplish SSS Form R-1 (Employer Registration) and R-1A (Employment Report) for the issuance of an Employer Number which shall be used in all transactions with regard to his/her household helper.


INSTRUCTIONS/REMINDERS


FOR SSS Form R-1:


1. Fill out this form in two (2) copies and accomplish appropriate parts as follows: 



  • For Employer Main Office - Parts I and III 
  • For Employer Branch Office - Parts II and III 

2. Submit this form to the nearest SSS office with accomplished Employment Report (SS Form R-1A)Specimen Signature Card (SS Form L-501), and a sketch of your business address.

3. The form shall be supported by applicable required documents and signed by authorized signatories:


  •  If Main Office

4. Pay the fee of P165.00 for Employer Registration Plate at the SSS or at any SSS-accredited bank and submit validated Miscellaneous Payment Return (SS Form R-6) or SS Form R-6 and Special Bank Receipt with this form. 

5. Notify SSS of any changes in data and the status of the employer's business operations to avoid being billed for period/s when no contributions are due. Fill up and submit Employer Data Change Request (SS Form R-8) supported by the required document/s, in any of the following cases: 

  • Temporary suspension, permanent cessation or merger/consolidation of business operations; and 
  • Changes in the employer's data such as business name, address, ownership, legal personality and other relevant information 
6. Request for replacement of the Employer Registration Plate, in case of loss or change of business name and/or address that will result to change of SSS branch ownership, using the Application for Employer Registration Plate.

7. Always use your 13-digit Employer Number in all your transactions with the SSS.


For SSS Form R1-A:


1. Submit in two (2) copies and indicate the types of employer and report by putting checkmarks on the applicable boxes. 


2. Employers are required to report within thirty (30) days to the SSS all its employees, who are subject to compulsory coverage. Should such employee die, becomes sick or disabled or reaches the age of sixty (60) without being reported by his employer, the latter shall pay to the SSS damages equivalent to the benefits due had he been reported on time. 


3. Misrepresentation of the true date of employment, monthly earnings or other data of employees is punishable under the penal provision of the SSS Law. 


         A. FOR REGULAR EMPLOYERS 

  • IF INITIAL REPORT 
    • Submit with the Employer Registration Form (SS Form R-1A) and the required supporting documents. 
    • The owner of a single proprietorship business is disqualified to be reported as an employee thereof. However, he may register as a self-employed member, provided he is not over 60 years old. 
    • Indicate the relationship to the owner, if a single proprietorship business reporting the owner's spouse, parent or unmarried minor child. 
  • IF SUBSEQUENT REPORT 
    • Submit with the unexpired Specimen Signature Card (SS Form L-501) signed by any of the authorized signatories in the SS Form L-501. 
    • Indicate your correct business name, 13-digit ER Number and business address as registered with the SSS. 
    • The owner of a single proprietorship business is disqualified to be reported as an employee thereof. However, he may register as a self-employed member, provided he is not over 60 years old. Indicate the relationship to the owner, if a single proprietorship business reporting the owner's spouse, parent or unmarried minor child.

  • The following family members of the Household Employer are not qualified for coverage as househelpers:
    • Parents 
    • Spouse 
    • Legitimate or illegitimate children/brother/sister/grandchildren/great grandchildren 
    • Parents/sisters/brothers-in-law 
4. Write "Nothing Follows" immediately after the last reported employee.

How to Register with SSS - For Voluntary Members

FOR VOLUNTARY MEMBERS

Who Are Covered?

1. Coverage of Separated Members
A member who is separated from employment or ceased to be self-employed/OFW/non-working spouse and would like to continue contributing.
2.  Coverage of Overseas Filipino Workers (OFWs)
A Filipino recruited in the Philippines by a foreign-based employer for employment abroad or one who legitimately entered a foreign country (i.e. tourist, student) and is eventually employed.
3.  Coverage of Non-working Spouses of SSS members
A person legally married to a currently employed and actively paying SSS member who devotes full time in the management of household and family affairs may be covered on a voluntary basis provided there is the approval of the working spouse. The person should never have been a member of the SSS. The contributions will be based on 50 percent of the working spouse’s last posted monthly salary credit but in no case shall it be lower than P1,000.
When does the coverage of members take effect?
Effectivity of Voluntary Coverage
  1. For an overseas Filipino worker – upon first payment of contribution, in case of initial coverage.
  2. For a non-working spouse – upon first payment of contribution.
  3. For a separated member – on the month he/she resumed payment of contribution.

Steps and Requirements:

Separated Members
A member who is separated from employment or ceases to be self-employed/OFW/non-working spouse may continue paying contributions through SSS Form RS-5 using the previously assigned SS number. Posting of said payment will change the membership status from covered employee, self-employed, OFW or non-working spouse to a voluntary paying member.

Non-Working Spouses
A non-working spouse should accomplish SSS Form NW-1 (Non-Working Spouse Record) and submit it, duly signed by the working spouse, with a copy of their marriage certificate.

Overseas Filipino Workers (OFWs)
An OFW should accomplish SSS Form OW-1 (Overseas Worker Record) and submit it together with a photocopy of any of the following: birth certificate, baptismal certificate, driver’s license, valid passport, Professional Regulation Commission (PRC) Card, or Seaman’s Book. In the absence of these documents, any two of the secondary documents mentioned in the section for employed and self-employed members will do.

FOR SELF-EMPLOYED INDIVIDUALS

Who are covered?

A self-employed person, regardless of trade, business or occupation, with an income of at least P1,000 a month and not over 60 years old, should register with the SSS. Included, but not limited to are the following self-employed persons.
a.
self-employed professionals;
b.
business partners, single proprietors and board directors;
c.
actors, actresses, directors, scriptwriters and news reporters who are not under an employer-employee relationship;
d.
professional athletes, coaches, trainers and jockeys;
e.
farmers and fisherfolks; and
f.
workers in the informal sector such as cigarette vendors, watch-your-car boys, hospitality girls, among others.


A self-employed person should accomplish SSS Form RS-1 (Self-Employed Data Record) and submit it together with a photocopy of any of the following:


  • Baptismal certificate
  • Birth certificate
  • Driver’s license
  • Passport
  • Professional Regulation Commission (PRC) Card, Seaman’s Book. 
You can get the SSS Form RS-1 at any SSS office or branch, or you can click here to download the form so you can just print it.

In the absence of these documents, ANY TWO of the following secondary documents, one of which with recent photo and date of birth, are acceptable:
    • Alien Certificate of Registration
    • ATM card with the cardholder’s name
    • ATM card and certification from the issuing bank that the account number belongs to the cardholder, if the card does not bear the cardholder’s name.
    • Bank Account Passbook
    • Birth/Baptismal Certificate of children
    • Certificate form
      • Office of the Southern/Northern Cultural Communities
      • Office of Muslim Affairs
    • Certificate of Naturalization issued by the Bureau of Immigration
    • Company ID
    • Company Representative Authorization Card issued by SSS

FOR NEW EMPLOYEES

It is compulsory for all prospective and/or new employees to register under SSS. This will also ensure that you get the benefits as a member of the Philippine society, in cases such as maternity, sickness, calamity and others.

When I had to go thru D&C (raspa due to blighted ovum), I got a whopping P30,000 for my maternity benefit! It was such a huge help since hospitalization is not that cheap. The benefits, though are based on how much your contributions are and how long you have been a contributing SSS member.

Who are covered?

  • A private employee, whether permanent, temporary or provisional, who is not over 60 years old.
  • A household-helper earning at least P1,000 a month is compulsory covered starting Sept. 1, 1993.
    • A household-helper is any person who renders domestic services exclusively to a household such as a driver, gardener, cook, governess, and other similar occupations.
  • A Filipino seafarer upon the signing of the standard contract of employment between the seafarer and the manning agency which, together with the foreign ship owner, act as employers.
  • An employee of the foreign government, international organization or their wholly-owned instrumentality based in the Philippines, which entered into an administrative agreement with the SSS for the coverage of its Filipino workers.

Registering as a member under SSS is very easy, but you have to be patient as there are a lot of people lining up for registration. I, personally, registered at the Makati - Gil Puyat Branch and it wasn't that crowded, same goes with the Manila branch near SM Manila. I think the main branch in Quezon City is too crowded so I won't suggest that you go there. For a list of SSS offices or branches, just go to www.sss.gov.ph and click on the drop-down button under "Branch Directory", as shown in the picture below:
  • Accomplish SSS Form E-1 (Personal Record) in two (2) copies and submit at any SSS office or branch.
    • You can get this form at any SSS office or branch.
    • You can also download this online and print it (make sure to cut along the dotted line.   • You need to have at least one (1) month of SSS contribution before you can apply for an SSS ID or UMID.
Once you have submitted all the requirements, the SSS personnel will give you your SSS number and you are officially a member of the SSS! Yey!
